熊去氧胆酸胶囊(优思弗)

【药代动力学】

1.吸收：优思弗口服后主要在空肠上段以非离子被动扩散方式，在回肠以主动转运方式吸收。UDCA的吸收与胆汁淤积的严重程度（以血清胆红素水平为判断标准）呈负相关。首过效应大，口服剂量的50-75%被肝脏摄取。2.分布：吸收后主要分布于肝肠组织和血浆中，血浆蛋白结合率为96-99%。优思弗的疗效主要取决于胆汁中UDCA的浓度。按10-15mg/kg/d服用UDCA后，胆汁中UDCA的浓度呈剂量依赖性升高，直至到达一个平台。胆结石患者胆汁中UDCA可占胆汁酸的40-60%；患有肝脏疾病的患者因十二指肠胆汁中胆汁酸胶粒缺乏，吸收减少而胆汁中UCDA所占的比例稍低。能透过血-脑屏障，体外实验证明，由于UDCA能抑制星型细胞凋亡而可能对患有肝脏疾病患者的脑细胞具有保护作用。3.代谢和排泄：在肝脏，UDCA与甘氨酸、牛磺酸、N-乙酰葡萄糖胺、葡萄糖醛酸和硫酸盐结合，在结肠，UDCA也可以被肠道细菌代谢。代谢产物小部分由尿道排出，大部分经粪便排出。口服UDCA生物半衰期为3.5-5.8天。
