丙酸氟替卡松吸入气雾剂(辅舒酮),急性吸入高于推荐的剂量可导致暂时性肾上腺功能抑制。那么,辅舒酮的药代动力学是什么?

辅舒酮(丙酸氟替卡松粉雾剂)

辅舒酮的药代动力学:

在对健康志愿者进行的为期14天的试验中,吸入剂量为每日2000ug(每次1000ug,每日2次),给药30-60分钟后,药物血浆浓度约为0.3ng/mL。对健康志愿者吸入给药后,丙酸氟替卡松全身生物利用度因剂型不同为10-30%。由于被吞咽的吸入剂量经消化道后所产生的生物利用度几乎为零,所以药物全身的吸收可由药物到达肺部的量来决定。丙酸氟替卡松与其它吸入糖皮质激素在哮喘治疗的药代动力学和药效学方面具有许多相似的特性。但与其它同类皮质激素相比,由于其不完全的胃肠道吸收和显著的首过效应,吸入后的丙酸氟替卡松经吞咽进入系统循环的几乎为零。对人类志愿者口服放射性标记和未标记的丙酸氟替卡松的研究表明,根据所用剂量不同,大部分剂量(87-100%)自粪便排泄,其中多达75%为原形药物。1-5%的剂量以代谢物形式经尿液排泄。对健康志愿者口服16mg单剂量药物,产生的血药浓度小于0.5ng/mL。健康志愿者单剂静脉注射2mg药物,显示丙酸氟替卡松的清除率与肝血流(900mL/分)速度接近,而肾清除率0.11mL/分)不到1%。这些结果表明肝清除几乎是完全的。口服生物利用度接近于零。静脉注射给药时,丙酸氟替卡松的药代动力学与给药剂量成正比。丙酸氟替卡松在身体内广泛分布。稳态分布体积接近300L,其清除率高达1.1L/分,提示广泛地肝清除。丙酸氟替卡松的血浆峰浓度在3-4小时内减少近98%,且在终末半衰期(约8小时)后,血浆中药物的浓度很低。在动物和人类试验中,抛射剂HFA-134a经呼吸迅速清除,未见在人体中有明显的代谢或蓄积。因达峰时间(tmax)和平均残留时间均极短暂,所以HFA-134a在血浆中停留时间短而没有蓄积。
