盐酸氯米帕明片(洞庭)

盐酸氯米帕明片的主要代谢途径是通过去甲基化形成活性代谢物N-去甲氯米帕明。N-去甲氯米帕明可由多种P450同功酶生成,主要是CYP3A4,CYP2C19和CYP1A2。盐酸氯米帕明片和N-去甲氯米帕明经羟基化,形成8-羟基氯米帕明或8-羟基-N-去甲氯米帕明。8-羟基化代谢物在体内的活性并不明确。盐酸氯米帕明片也可在2位发生羟基化,而N-去甲氯米帕明还可以进一步去甲基化形成双去甲氯米帕明。2位及8位羟基化的代谢物主要以葡萄糖醛酸化物的形式从尿液中排出。盐酸氯米帕明片的活性成份(氯米帕明和N-去甲氯米帕明)以形成2位和8位羟基化氯米帕明的方式进行清除,此过程是由CYP2D6进行催化。

盐酸氯米帕明片97.6%的与血浆蛋白结合。盐酸氯米帕明片的表观分布容积约为12-17升/公斤体重。盐酸氯米帕明片在脑脊液的浓度相当于血浆浓度的约2%。盐酸氯米帕明片进入母乳的氯米帕明的浓度与血浆中的浓度近似。盐酸氯米帕明片在血液中清除的半衰期平均为21小时(范围:12-36小时),而去甲氯米帕明的平均半衰期为36小时。盐酸氯米帕明片单次给药后,约2/3以水溶性结合物的形式从尿液中排出,约1/3从粪便中排出。盐酸氯米帕明片从尿液中排出的原型和去甲氯米帕明的量分别占服用剂量的2%和0.5%。

盐酸氯米帕明片与抗惊厥药合用,可降低抗惊厥药的作用。
